These guidelines provide advice for managers in trusts and breast screening services on investigating a suspected incident in a breast screening unit and the steps to be taken if an incident is confirmed. They replace the previous edition published in 1999 and take account of organisational changes to the NHS and the development of the system being developed by the NPSA for reporting patient safety incidents.
